Proper Identification – Onboarding for an Examination
It is important to keep your profile details correct and up to date in the Credentia Platform to ensure trouble-free exam onboarding and accurate certificate generation.
Please review your profile details and make sure all information is correct and updated before scheduling your exam or while applying for certificate renewal.
In the Manage Profile section on the Credentia Platform, you can correct or update your profile details, including your current address, phone number, and gender details.
What types of ID are accepted?
It is important to familiarize yourself with the ID requirements specific to your state by reviewing the candidate handbook. Go to credentia.com > click on Find My Exam > Select the state from the list > scroll down to the Resources section > click on Candidate Handbook to locate the necessary ID requirements for your state.
In most states, you are required to bring two (2) forms identification that are government issued, official, and signature-bearing identification to the test center, or have with you at the time of your online exam (one of which must be a photo identification). All identification must be current (not expired), and birth certificates will not be accepted. Photocopies of identification will not be accepted. Examples of proper identification include:
- Driver’s license
- Signature bearing social security card
- Clinic card
- Credit card
- Library card
- State-issued identification card
- Passport
- Alien registration card
What is an ID card mismatch?
The name on your government issued photo identification (ID) card and any other form of ID must exactly match the name on your application submitted for examination registration.
You should verify that your ID cards are valid and do not expire before the scheduled day of your examination.
If your name has changed due to marriage, divorce, or other reasons, you MUST update the official ID card with the name change in the application before taking the test.
Can I take the test without my ID card?
If you do not provide the required forms of original proof of identification, you will not be allowed to take the test.
You are required to bring a valid, non-expired, government-issued photo ID. Photocopies of identification will NOT be accepted, and your examination fee will not be refunded.
