Alabama (NNAAP)

You are required to bring two (2) forms of current, official, signature- bearing identification to the test site (one of which must be a photo identification). All identification must be current (not expired) and no birth certificates will be accepted. Photocopies of identification will NOT be accepted. Examples of proper identification include:

  • Driver’s license
  • Signature-bearing Social Security card
  • Clinic card
  • Credit card
  • Library card
  • State-issued identification card
  • Passport
  • School-Issued Identification Card.

The name on your identification must be the same as the name you used on the application to register for the examination. If your name is different, you MUST bring proof of your name change (a copy of an official document such as a marriage license or divorce decree) to the test center. If you do not bring proper identification, you will not be allowed
to test, and your examination fee will not be refunded.

Alabama MAC

Candidate must present two (2) forms of current (not expired) signature identification.

The name on the identification must exactly match the name on the registration. The primary identification must be government issued, photo-bearing with a signature and the secondary identification must contain a valid signature. Identification must be in English.

Photocopies of identification will NOT be accepted.

Primary ID (photograph, and signature, not expired)

  • Government-issued Driver’s License
  • U.S. Department of State Driver's License
  • U.S. Learner’s Permit (plastic card only with photo and signature)
  • National/State/Country ID card
  • Passport
  • Passport Card
  • Military ID
  • Military ID for spouses and dependents
  • Alien Registration Card (Green Card, Permanent Resident Visa)

Secondary ID (signature, not expired)

  • U.S. Social Security Card
  • Debit (ATM) card or Credit card
  • Any form of ID on the Primary list

If the ID presented has an embedded signature that is not visible (microchip), or is difficult or impossible to read, the candidate must present another form of identification from the Primary ID or Secondary ID list that contains a visible signature. Credentia does not recognize grace periods. For example, if a candidate’s driver’s license expired yesterday and the state allows a 30-day grace period for renewing the ID, the ID is considered to be expired.

Alaska

You are required to have two (2) forms of official ID with a signature (one must be a photo identification). Photocopies of identification will NOT be accepted. Examples of identification you can use include:

  • Driver’s License
  • Social Security Card
  • Clinic Card
  • Credit Card
  • Library Card
  • State-issued Identification Card
  • U.S. Passport
  • Tribal ID (With Signature)
  • Military Identification

The name on your identification must be the same as the name you used on the application to register for the examination. Fees are not refundable. If you do not bring proper identification, you will not be allowed to test, and your examination fee will not be refunded. If you no show for two examinations, your application to test will be removed and you will need to re-apply.

California

You are required to bring two forms of official, signature-bearing identification. 1) Your original Social Security or ITIN card. And 2) A legal photo identification that includes signature. Photocopies of identification will NOT be accepted. Examples of legal photo identification include:

  • Driver’s license
  • Signature Bearing Social Security Card/ITIN card
  • DMV Identification card
  • Passport or Passport card
  • U.S. Military card

The first and last names or suffix (i.e., Jr., II, III), on the current government photo identification, social security card, Nurse Assistant Certification (HS 283b), or CDPH Approval letter (CDPH 923) must be the same name used on the registration application to register for the examination. If you do not bring proper identification, or if your names do not match on all presented documents, you will not be allowed to test, and your examination fee will not be refunded. All documents presented at test site must be current, original, legible, and unaltered.

Colorado
(NNAAP &
MACE)

Two (2) forms of current, not expired, signature-bearing identification. One MUST be a government-issued photo identification (for example: driver’s license). Please note: THE SIGNATURES ON THE TWO (2) IDs MUST MATCH.
If you come to the test site without the proper ID, you will not be allowed to take the exam and you will lose your exam fee. Examples of proper identification include current, not expired:

  • Driver’s license
  • Signature-bearing Social Security Card
  • Military Identification
  • Clinic Card
  • Credit Card
  • Library Card
  • State-issued identification card
  • US Passport

The name on your identification must be the same as the name you used on the application to register for the examination. If your name is different, you will not be allowed to test.

District of
Columbia
(NNAAP &
HHA)

Get your ID ready – you are required to have two (2) forms of official ID with a signature (one must be a photo identification). Photocopies of identification will NOT be accepted.

  • Credit card

  • State-issued identification card
  • Passport

IMPORTANT: If you can’t take the test because of this, it still counts as one of your 3 chances to take the exam because you were not prepared — please have the correct forms of identification!

DoDEA

Candidates are required to bring two (2) forms of current, not expired official government-issued signature bearing identification (one of which must be photo-bearing) to the test site. Photocopies of identifications will not be accepted.

  • One form of identification must be a U.S government issued Social Security (SS) card, signed and not laminated.
  • A second form of identification must be any of the items listed below and must be photo-bearing.
  • Current, non-expired driver’s license
    • Candidates who do not have their new license by exam day must bring their expired driver's license and their temporary permit.
  • Current, non-expired learner’s permit
  • Military Identification
    • Sponsor military identification and a dependent military identification may be used when presented with a valid SS card.
  • State - or federal - issued identification card.
  • Passport (US or foreign, current, non-expired)
  • Alien registration card
  • Employment authorization document (EAD) photo identification card (current, non-expired, federal-issued)

The name on your identification must be the same as the name you used on the application to register for the examination.

If you do not bring proper identification, you will not be allowed to test, and your examination fee will not be refunded. This absence will count as one of your 3 opportunities to test because you were not prepared with the correct forms of identification.

Georgia

Candidates you are required to have two (2) forms of official ID with a signature (One form of identification must be a U.S. government issued Social Security (SS) card, signed, and not laminated.). A second form of identification must be any of the items listed below and must be photo- bearing U.S. Government-issued driver’s license.

  • U.S. Government Issued Driver’s license
  • State-issued identification card
  • Passport
  • Georgia- Issued Voter ID Card
  • Georgia Temporary Driver’s License
  • Signed Photo High School I.D. (for high school students)
  • Signed Photo University I.D. (for university students)

Maryland

Carolina

If a Candidate fails to present proper identification, then he or she will not be allowed to take the exam and the exam fee will not be refunded.
The name listed on the identification must be the same as the name used to register for the exam, including suffixes and hyphens. If your name has changed, then you must complete the steps below before your scheduled exam. Be prepared to provide official government documentation to your name change.

BEFORE TESTING: Log into your Credentia Platform and submit a Demographics
application. There, you will be required to upload the appropriate documentation as related to the information you wish to update or change.

North
Carolina
(NNAAP, MAAC & MA-NH)

Identification Requirements
To take the NNAAP® examination, you must bring two (2) forms of current, unexpired, official, signature-bearing identification to the test site. One of these forms must be a photo ID. For Exceptions, please refer to the Note section below. Your name must appear exactly on the Credentia platform as it appears on your provided identification.

Required Identifications

  1. Social Security Card
    • Must be issued by the U.S. government.
    • Must be signed.
    • Laminated cards will not be accepted.
  2. One Photo ID from the List Below:
    • Driver’s license
    • Learner’s permit
    • Military Identification (sponsor or dependent)
    • State- or federal-issued ID card
    • U.S. or foreign passport
    • Federal-issued Employment Authorization Document (EAD) photo ID

Please Note:

  1. Per North Carolina General Statute 20-7(f)(5), temporary licenses, permits, or identification cards issued by the North Carolina Division of Motor Vehicles (DMV) will be accepted for 60 days from issuance. Official DMV paper copies will be accepted. This does not apply to temporary licenses, permits, or identification cards issued in other states.
  2. Per North Carolina Session Law 2025-47 (Senate Bill 391), Class C licenses (standard passenger vehicle licenses) expiring on or after July 1, 2025, will be accepted for two years beyond the printed expiration date. The North Carolina Division of Motor Vehicles will allow this process until December 31, 2027. This does not apply to commercial driver licenses, or any licenses that are suspended, revoked, or cancelled. This does not apply to licenses issued in other states.

Additional Guidelines

  • Photocopies of any identification will not be accepted.
  • If presenting sponsor or dependent military ID, it must be accompanied by a valid Social Security card.

The name on your identification must exactly match the name you used on your examination application — including suffixes, hyphens, or other punctuation.
If a candidate fails to present proper identification, he or she will not be eligible to take the exam.
The name listed on the identifications must be the same as the name used during the registration process. If your name has changed, then you must complete the steps below at least nine (9) calendar days before your scheduled exam. Be prepared to provide official government documentation of your name change.

Virginia

You are required to have two (2) forms of official ID with a signature (one must be a photo identification). Photocopies of identification will NOT be accepted. All identification must be current (not expired), and no birth certificates will be accepted. 

Examples of proper identification include:

  • Driver’s license
  • Social Security card
  • Clinic card (example — Medicaid ID card)
  • Credit card
  • Library card
  • State-issued identification card
  • US Passport
  • United States Uniformed Services ID with photo
  • High School ID with photo
  • Signed High School ID card (must have signature line)
  • University ID with photo
  • Signed University ID card (must have signature line)

The first and last name on your identification must be the same as the first and last name you used on the application to register for the examination. If your first or last name is different, you MUST bring proof of your name change (a copy of an official document such as a marriage license or divorce decree) to the test center.

Washington
(NNAAP &
MACE)

Candidates are required to bring ONE (1) form of current, not expired, official photo identification card.

  • It does NOT need to be signature bearing.
  • Photocopies of identification will NOT be accepted.
  • Proper identification include:
    • Driver’s license
    • State issued learners permit
    • State issued identification card
    • School issued identification card
    • US Passport
    • US Passport Card
    • Tribal ID Card with photo

The name on your identification must be the same as the name you used to register for the examination, including suffixes. If your name has changed, you must log on to your account at least nine (9) calendar days prior to either scheduled exam, and follow the instructions you are given there. Be prepared to provide official government documentation of the name change.

Kentucky

Candidates are required to bring two (2) forms of current, not expired official U.S. government-issued signature-bearing identification (one of which must be photo-bearing) to the test site. Photocopies of identifications will not be accepted.

You must present two forms of government-issued identification on exam day.

Here’s what’s required:

  • One ID must have your photo.
  • Both IDs must have your signature.

Acceptable Forms of Identification:

  • Employment Authorization Document (EAD)
  • Veteran ID Card
  • Native American Tribal Photo ID

  • State-Issued Learner’s Permit
  • Driver's License or ID from Puerto Rico, Guam, U.S. Virgin
    Islands, American Samoa, or Northern Mariana Islands

Official Digital driver’s licenses are acceptable if presented digitally, but one form of ID must be paper (non-laminated). Digital drivers licenses are provided through mobileID

If your name has changed, you must log on to your account at least nine (9) calendar days prior to either scheduled exam, and follow the instructions you are given there. Be prepared to provide official government documentation of the name change.

Tennessee

You are required to bring two (2) forms of unexpired official, signature-bearing identification to the test site (one of which must be a photo identification, the other, your social security card). Digital or Photocopies of identification will NOT be accepted. 

Examples of proper identification include:

  • State Issued Driver’s license 
  • Social Security card (must not be laminated)
  • Permanent Resident Card
  • Concealed Hand Gun Carry Permit
  • Military ID card 
  • State-issued identification card 
  • Signed US Passport
    • Exception: A signed foreign passport with a US VISA within the passport is acceptable (the VISA does not have a signature).

Utah
