How to Apply for Accommodations?

Apply for accommodations while submitting the testing application.

Test accommodations are individualized and considered on a case-by-case basis. All candidates requesting accommodation must provide the appropriate documentation of their condition and how it is expected to affect their ability to take the test under standard conditions. This may include: 

  • Supporting documentation from the professional who diagnosed the condition, including the credentials that qualify the professional to make this diagnosis.
  • A description of past accommodation the candidate has received.

Note: Credentia complies with the amended provisions of the Americans with Disabilities Act. The purpose of accommodation is to provide candidates with full access to the test. Credentia provides reasonable and appropriate accommodation to individuals with documented disabilities who demonstrate a need for it.

The following are the steps to apply for accommodations online in the Credentia Platform: 

Step 1: Log in to the Credentia platform. 

  • Log in to the Credentia Platform using the registered login credentials. Visit the credentia.com website, navigate to the login page, and enter your registered email address and password to log in.

Step 2: Navigate to the Application 

  • Navigate to the application or dashboard section using the side menu. Then, click on the "Start New Application" button.
  • Note: If you already have an approved application without applying for the accommodation, you need to contact Credentia support at credentia.com and click the Chat Support icon at the bottom right to connect with us.

Step 3: Selecting the accommodation option while filling out an application form. 

  • After selecting your eligibility route, you will be directed  to the application to complete.
  • Select Yes in the accommodation section, and an Accommodation Form will appear at the bottom. 

Step 4: Completing the Accommodation Form 

  • Select  the options that apply to your accommodation, fill in the necessary details, and upload any required documents to continue to the next step.
  • All mandatory sections must be completed before submitting the application.

Step 5: Submitting the Form.

  • After you have completed the application form, click Submit to send for review.

In order to enroll or update your eligibility route for your nurse aide testing and certification, you must submit the application along with your accommodation form.

Additionally, the application may require details of the training program, military information, and other declarations, if applicable.

Next steps: The submitted application with accommodation will go through an application review process, and you will be notified of updates or requests via your registered email address.
